The purpose of this study being conducted by Sarah Charnock, under the supervision of Dr. Colin King (Assistant Professor and Certified Athletic Therapist), is to determine the effectiveness of sports related concussion management in Nova Scotia. It includes a short questionnaire and an optional follow up interview.
As a participant, you will be asked to complete a questionnaire outlining your knowledge of and experience with concussion management. The questionnaire will take between 15-20 minutes to complete. Upon completion of the questionnaire you will be asked to leave your contact information if you are willing to participate in the interview portion of the study at a later time (interviews will be scheduled one month after the completion of the questionnaire). Interview questions will be established based on the results of the questionnaires once they have been analyzed. Your contact information will be kept confidential and all contact details will be deleted once the interview has taken place. All data collected will be kept on a password protected computer that is only available to the principle researcher and her supervisor.
Participation in the study is completely voluntary and your consent can be withdrawn at any time during the interview and up to 30 days afterwards however responses to the questionnaire may not be removed once submitted.There are no known risks associated with the study. Data from this study may be used for the creation of an individualized concussion management program to be used at Acadia.
Once the questionnaires have been submitted, you will be unable to withdraw your results as they will be automatically and anonymously added to the bank of group data. Since the questionnaires are anonymous, you will not be linked to your questionnaire answers.

Consent
The purpose of this study is to explore athlete’s concussion management experience and understand the barriers that prevent effective management of sports related concussions. The nature of your participation has been described above. If you decide to participate, the confidentiality of your participation will be ensured as previously detailed. Consenting to participate in this research does not waive your rights to legal recourse in the event of research-related harm.
